Discussing failure isn't always easy, nor politically correct - yet it's essential in understanding how we can do things better in future implementations.

Author Dani Barrington looks at some examples in her article "Blunders, Bloopers and Foul-Ups: Sharing Failures in Water, Sanitation and Hygiene Programs" - from incorrect use, accidental drowning to culturally incompatible, there are a few interesting case studies to look at.
